Missouri Slope was awarded a grant from Lutheran Services of America to address the increasing number of underserved and isolated older adults in rural communities experiencing difficulty accessing services and supports to successfully age at home.
With deep understanding of what older adults need in rural communities, Missouri Slope has developed a new program called Missouri Slope at Home to provide a line of in home service that enable older adults to remain in their home.
Missouri Slope at Home has also created a partnership with other area providers to create the Rural Aging Action Network (RAAN) to collaborate and address the needs of older adults in our rural communities.
Missouri Slope at Home exists to empower older adults in our region to age with independence, dignity and purpose in the community of their choice. Our program allows aging adults to get the assistance they need to live happily, healthy, and safely, while remaining in the home they love.
Following a consultation with a member of our team, we will customize a home care plan based on individual needs and preferences.
Our caregivers are ready to help you, your family member, friends or neighbors to age happily and safely in the comforts of home while providing peace of mind for loved ones.
Services are available individually or in preset packages which can be paired with other services based on individual need.
Missouri Slope at Home Individual Services
Meal Delivery
Made fresh at Missouri Slope, Frozen and delivered upon order.
Service Coordination
If additional services are needed, we coordinate with other providers to put services in place.
Safety Checks
Contact through a call up to 5 times per week to ensure all is well.
Referral services
If we don’t offer what you need, we connect you to other providers.
Transportation
We provide medical and non-medical transportation paired with other services.
Companionship
We provide an individual to spend time with you in or out of the home.
Housekeeping
Light household tasks including sweeping vacuuming, dusting, laundry.
Personal Care
Assist with items you struggle with; dressing, bathing, showering, hair care.
Missouri Slope At Home Packages
Services are available in preset packages which can be paired with other services based on individual need.
- All services and packages are non-medical.
- Our Personal Care attendants are unable to provide skilled nursing level care.
- Services and Packages are private pay only or we accept Long Term care insurance if applicable.

Our service delivery area includes Burleigh, Morton and McLean counties, especially rural communities that may not otherwise have access to such services and supports.
Availability of services is subject to available staff and service delivery area.
Who is a Good Fit for Missouri Slope at Home?
Our program has been designed to assist older adults live independently in the place of their choosing for as long as possible. If you experience one or more of the following, Missouri Slope at Home Services may be a good fit;
- Struggles with mobility and need assistance safely moving throughout the home
- Impaired motor skills who need assistance with daily household tasks
- No longer drive and need transportation to the grocery store, friends’ houses, and more
- Feelings of isolation and desire the companionship of a caregiver
- Seniors who need assistance with activities of daily living, including meal preparation, bathing, and more
- Unable to take care of basic housekeeping and home chores or home maintenance
Reasons why older adults may not be a good fit for Missouri Slope at Home
- You need/ would benefit from 24 hour access to care, nursing care, or other regular medical care
- You want an all-inclusive lifestyle like those offered in many residential care communities

Home and Community Based Services (HCBS) / Medicaid Covered Services
If you need financial assistance to pay for care or services, as a qualified service provider (QSP), Missouri Slope at Home can still help.
You must apply for services through the ND Department of Human Services, Aging and Disability Resource Link (ADRL) by calling: 1-855-462-5465. Once qualified, you may request Missouri Slope at Home as your Qualified Service Provider.

Addressing Community Needs
In effort to develop a program that addresses the needs in rural communities, specifically rural Burleigh, Morton, and McLean counties, Missouri Slope at Home has developed a survey to help us determine the greatest areas of need.

The survey results are anonymous. Data will be used for the purpose of strengthening home and community-based services in the areas of: transportation, access to healthy meals, access to healthcare, safe housing and socialization. Thank you for participating.
Rural Aging Action Network (RAAN)
The RAAN is a network of partners in Burleigh, Morton and McLean counties, and run by Missouri Slope at Home. Our partners, either play a role or have the ability to impact the lives of aging adults to remain in the home of their choosing.
The RAAN meets monthly and is collaborating to:
- Address barriers to senior care
- Strengthen caregiver’s capabilities and pipelines
- Optimize sustainable solutions in rural communities

Missouri Slope at Home Meal Delivery Program
The goal of Missouri Slope At Home’s Meal Delivery Program is to ensure aging adults have access to healthy, affordable meals.
- Meals are delivered on Monday Mornings.
- All meals are made fresh, then directly packaged & sealed in our Missouri Slope kitchen with a variety of meals will be delivered each week.
- Each meal includes protein, starch, & vegetable. (Casseroles/hotdishes may be inclusive of all three.)
- 5 per order minimum for delivery.
- Missouri Slope at Home will deliver within a 30 mile radius of Bismarck-Mandan.
- Individual meals cost $10. For every 7 meals ordered in one week, receive a $5.00 discount (7 for $65).
- Payment due at the time of delivery.
- Cash or check acceptable.
- Must Be 55 years of age or older.
